Grievances, Exceptions and Appeals Process:

Physician-Dispensed Vaccines:

If you have been prescribed a vaccine that is covered under Medicare Part D that was dispensed and/or administered by your doctor, your doctor may submit claim(s) for the vaccine as well as the vaccine administration fee to SilverScript for reimbursement and collect only your co-payment by submitting the Medicare 1500 form to:
